As-Needed Medications

What are as-needed medications?
Some psychiatric medications are taken only when a specific symptom or situation arises, rather than every day. These options may be useful for concerns such as physical symptoms of anxiety, alcohol cravings in anticipated drinking situations, or short-term difficulty with sleep.
Beta Blockers
Beta blockers, such as propranolol, may be used to reduce the physical symptoms of situational anxiety, including a racing heart, shakiness, or trembling. They are sometimes considered for performance-related anxiety, such as public speaking or presentations, when the body’s stress response is the main concern.
Targeted Naltrexone
Naltrexone is an FDA-approved medication for alcohol use disorder. In some cases, it may be used in a targeted way, taken before anticipated drinking, to help reduce cravings, heavy drinking, or the rewarding effects of alcohol. This approach is considered on an individual basis and may be appropriate for people whose goal is to reduce alcohol use rather than pursue immediate abstinence.
Sleeping Aids
When sleep difficulties are intermittent, certain as-needed sleep medications may be considered. The choice depends on the nature of the sleep problem, such as difficulty falling asleep, staying asleep, or occasional periods of heightened stress that disrupt rest. Medication is selected carefully, with attention to safety, next-day effects, and the broader causes of insomnia.
Are as-needed medications right for everyone?
Not necessarily. These medications are used thoughtfully and only when they fit a person’s symptoms, medical history, and treatment goals. They may be part of a broader plan that also includes therapy, behavioral strategies, or daily medication when appropriate.
